In order for us to pursue Recoveries on your behalf, we often require certain "procedural" documents in addition to claims documents.

To assist you and/or your insured, we have provided certain document wordings on this page.

Please select the document required - Subrogation, Letter of Assignment, Letter of Authority. You may either download the image as a .pdf file which you may print immediately onto your letterhead or download a word document. This will download the respective document text for you to complete or modify as required, again on your own letterhead paper. Please return the original of the completed document to us by mail. We would appreciate an advance copy by fax.
